Happy Valley Family Health Team

The Happy Valley Family Health Team is a dynamic team of professionals that provide comprehensive care to residents of St. Marys and the surrounding rural area. The multi-disciplinary team consists of physicians, nurse practitioners, nurses, dietitian, pharmacist, mental health worker, addictions counsellor and administrative support. The team is committed to providing primary healthcare services to the community that are innovative, collaborative and interprofessional.

 

A mask or face covering MUST be worn during your visit to our clinic.  We are unable to provide a mask to all patients.  You are expected to come wearing a mask or non-medical grade face covering of your own.  Click the link for information on how to make your own face covering, as well as how to safely wear a non-medical grade face covering.  A mask or face covering should not be worn by children under the age of 2 or by anyone with a medical condition that prevents them from safely wearing a mask.  If your child is between the ages of 2-5 years old and you feel that they can safely wear a mask or face covering, we encourage this.

FLU SHOTS

We will not be providing the Flu Shot at our Clinic this year, instead we have partnered with the Town of St.Marys to provide the flu shot to Residents of St.Marys and surrounding rural area.  NO WALK INS.  

Must call 519-284-3450 ext. 3235 to BOOK an APPOINTMENT between the hours of 9-12, 1-4.  Please have your Health Card READY when calling to book.   No High Dose vaccine will be given at these clinics.
